Incident Investigation and Reporting
JRM has developed and implemented its own protocol for incident
reporting, investigation and follow-up of corrective and preventative
actions. The scope of the incidents covered by the protocol
includes near misses, equipment damage, employee injuries/illnesses,
and environmental releases. The intent is to focus on potential
severity rather than the actual severity of incidents so that
high risk situations will be identified and mitigated before
another (possibly more serious) incident occurs. Potential
severity relates to loss involving personnel, assets, environment,
and/or company reputation. All accepted recommendations are
entered into an action registry to ensure they are tracked
through completion.
Correcting Deficiencies
All operational deficiencies found as a result of audits,
inspections, incidents etc. are recorded and tracked to ensure
that they are addressed in a timely manner and that any preventive
measure and improvements are implemented, as necessary.
